@janekohara @QualityForum “Zero avoidable” an absolute determinist mindset laced with hindsight not congruent with complex adaptive systems. “Harm” is highly subjective in healthcare. As a strap line it’s not debatable. As a goal when understanding the messy reality- flawed at best.

Concerning at such a level decisions being abdicated to data in such a way. Context changes everything.
