BiTEs in myeloma: Why aren’t we talking enough about time limited therapy?
🕰️ Our 69 year old lady with heavily pretreated RRMM received 8 months of teclistamab → CR. Stopped in August 2025 due to recurrent infections.
🙌🏻14 months off therapy: Flow-MRD negative, normal myeloma profile, doing well!
➡️What does emerging evidence show?
🟠LimiTec (n=43): 77% and 73% failure-free probability at 6 and 12 months after stopping teclistamab.
🟠Vega et al., HemaSphere 2026 (n=81): 81% relapse-free at 12 months, 71% at 24 months after bispecific discontinuation; infections prompted discontinuation in 48%.
We have stopped bispecifics in 2 more patients, with shorter follow-up.
🛎️Not practice-changing yet, but compelling evidence to investigate finite therapy.
Infections, immune suppression, cost and quality of life matter.
Should MRD-guided discontinuation be our next frontier in myeloma, rather than indefinite bispecific therapy?

@MirghSumeet@RahulBanerjeeMD@GunjanLShah@ASTCT_Journal@bagalbp@PrashantTembha1 Only a randomised trial done between dvrd vs Drvd plus auto can give the real clue and mrd today means a ngs mrd. So for us 4 cycles of vrd and 8 doses of dara follwed by 200 mg /m2 of mel has almost given 92% of pt being alive at 6 yr
Does myeloma maintenance need to continue indefinitely?
I spoke with @AJMC_Journal's Maggie Shaw about ENDURANCE, lenalidomide maintenance and balancing disease control with toxicity, quality of life & MRD-guided care.
It's not just about living longer, but living better.

Blood cancer treatment is moving beyond conventional chemotherapy. Our conversations with patients need to reflect that.
Today, the family of a newly diagnosed myeloma patient came to our OPD. Feared starting “chemo.”
Hair loss. Mouth ulcers. Vomiting. That was the picture in their minds.
But Dara-VRd works differently. It combines targeted medicines, immune-based treatment and a steroid.
It contains no conventional chemotherapy drugs.
The side effects they feared are not inevitable.
This change extends beyond myeloma:
1. Follicular lymphoma: EHA guidelines out today highlight more chemo-free options, especially at relapse.
2. Ph-positive ALL: new NEJM update of GIMEMA ALL2820 study supports targeted medicine plus immunotherapy, without conventional systemic chemotherapy.
These treatments still have risks.Careful monitoring remains essential.
Treatment is evolving. The way we explain it should too.
It is an honour to welcome Dr Kojo S. J. Elenitoba-Johnson, Chair of the Department of Pathology and Laboratory Medicine at Memorial Sloan Kettering Cancer Center, USA.
We are delighted to have you with us, Sir.
