🚨 AN APPEAL FROM A GRIEVING FATHER FOR COMPLETE MEDICAL RECORDS FROM RAINBOW CHILDREN'S HOSPITAL, VIJAYAWADA (CURRENCY NAGAR BRANCH) 🚨
IMPORTANT NOTE: Posting this not to scare or blackmail anyone. As a grieving father, after numerous failed formal attempts to get my daughter's treatment records, I am forced to post this here as a last resort for higher authorities' attention.
My minor daughter, Baby D. Naga Vaishnavi (Patient ID: IP6-00035075), tragically passed away while undergoing treatment at Rainbow Children's Hospital, Vijayawada in Sept 2021. Even in that midnight hour of absolute trauma and devastating grief of losing my child, I acted with utmost responsibility, strictly followed your hospital protocols, and cleared lakhs of rupees in bills before taking her body. When I respected and abided by your hospital protocols during my darkest hour, why is the hospital management now refusing to follow official medical and government protocols to hand over my daughter's complete case sheets?
THE REASON WHY I NEED THESE RECORDS NOW:
I urgently need these files now because I am actively pursuing a legal court case and fighting for justice against the local hospital where my daughter was initially admitted before being shifted to Rainbow. These medical records are crucial supportive evidence for my ongoing court case. It is highly unfortunate that while I am fighting for legal justice, your Vijayawada branch is causing severe delays and withholding my legal evidence.
Since 10th March 2026, despite sending 6 to 7 official request letters via Registered Post and approaching the DMHO and the District Collector's Grievance cell, the branch continues to delay. Following explicit directives from authorities, the hospital finally handed over a case sheet file. However, that file is incomplete; critical medical records from the final 2 to 3 days before her tragic demise have been withheld and are missing. As a father, it is my absolute right to know exactly what specific treatments were administered during those crucial final days.
COMPLIANCE ISSUES NOTICED IN THE PROVIDED FILES (See Attached Photos):
The files handed over clearly reveal serious protocol violations regarding consents signed by my younger brother, Mr. Chanti Dharavathu (Patient's Attendant):

1. Blank Consent Forms: My brother's signatures were taken on multiple 'Special Procedures' and 'Sedation' forms, but critical fields like the procedure name and associated risks were left ENTIRELY BLANK. Under Supreme Court guidelines, such blank consents hold no legal validity.
2. Mixing of Records: A "High-Risk Informed Consent" form belonging to a completely different patient was mixed into my daughter's file and my brother was made to sign it as a witness.

Regarding these missing files, I personally met Unit Director Mr. Surya on 26th May 2026. He assured me that if records were unavailable, they would provide a clear explanation on the Hospital’s Letterhead. However, since that day, he has gone back on his word, blocked my number, and is completely ignoring my WhatsApp messages.
According to NMC regulations, a complete medical record is a fundamental patient right. I request Corporate Management and Chairman Dr. Ramesh Kancharla to kindly intervene, provide the missing records, and arrange a callback from a responsible Telugu-speaking executive.

🚨 విజయవాడ రెయిన్‌బో హాస్పిటల్ (కరెన్సీ నగర్) నుండి రికార్డుల కోసం ఒక తండ్రి ఆవేదనతో కూడిన విజ్ఞప్తి 🚨
గమనిక: నేను ఈ సమాచారాన్ని ఇక్కడ పెడుతున్నది ఎవరినీ భయపెట్టడానికో లేదా బ్లాక్‌మెయిల్ చేయడానికో కాదు. ఒక తండ్రిగా నా కుమార్తె చికిత్స వివరాలు తెలుసుకోవడానికి నేను చేసిన అనేక చట్టబద్ధమైన ప్రయత్నాలకు మీ విజయవాడ బ్రాంచ్ యాజమాన్యం నుండి సరైన స్పందన రాకపోవడం వల్లే, చివరి ప్రయత్నంగా ఇక్కడ పోస్ట్ చేయాల్సి వచ్చింది.
మీ రెయిన్‌బో హాస్పిటల్‌లోనే చికిత్స పొందుతూ నా కుమార్తె బేబి డి. నాగ వైష్ణవి (Patient ID: IP6-00035075) సెప్టెంబర్ 2021లో మరణించింది. ఆ అర్థరాత్రి వేళ కన్నపాపను కోల్పోయిన అంతటి ఘోరమైన గుండె కోతలో ఉన్నప్పటికీ, నేను ఎంతో మర్యాదగా మీ హాస్పిటల్ రూల్స్ అన్నీ పాటించి, లక్షలాది రూపాయల బిల్లులను పూర్తిగా చెల్లించి మరీ నా పాప పార్థివ దేహాన్ని తీసుకెళ్లాను. నా జీవితంలోనే అత్యంత చీకటి రోజున నేను మీ నిబంధనలను గౌరవించి పూర్తి పేమెంట్ చేసినప్పుడు, మరి ఇప్పుడు రోగి రికార్డులను పూర్తిగా అందించాలనే కనీస నియమాలను (Protocols) మీ హాస్పిటల్ యాజమాన్యం ఎందుకు పాటించడం లేదు? నా కుమార్తె కేస్ షీట్ ఎందుకు దాస్తున్నారు?
ఇన్ని సంవత్సరాల తర్వాత నేను ఈ రికార్డులు ఎందుకు అడుగుతున్నానంటే:
నా కుమార్తెను మొదట అడ్మిట్ చేసిన స్థానిక ఆసుపత్రిపై నేను ప్రస్తుతం కోర్టు కేసు (న్యాయపోరాటం) చేస్తున్నాను. �� కోర్టు కేసుకు మద్దతు ఇచ్చే బలమైన ఆధారంగా (Evidence) ఈ రికార్డులు నాకు అత్యంత అవసరమై అడుగుతున్నాను. నేను న్యాయం కోసం పోరాడుతుంటే, మీ విజయవాడ బ్రాంచ్ వారు కావాలనే కాలయాపన చేస్తూ నా కోర్టు ఆధారాలను ఇవ్వకుండా ఆపడం చాలా విచారకరం.
నా పాప కేస్ షీట్ కోసం మార్చి 10, 2026 నుండి నేను మీ హాస్పిటల్‌ను కోరుతున్నాను. యాజమాన్యానికి 6 నుండి 7 సార్లు అధికారిక రిజిస్టర్డ్ పోస్ట్ లేఖలు పంపాను. స్పందన లేకపోవడంతో DMHO మరియు జిల్లా కలెక్టర్ గ్రీవ��న్స్ సెల్ ని ఆశ్రయించాను. అధికారుల ఆదేశాల తర్వాత మీ హాస్పిటల్ వారు నాకు కేస్ షీట్ ఇచ్చారు. కానీ అందులో పాప చనిపోవడానికి ముందు చివరి 2 నుండి 3 రోజులకు సంబంధించిన రికార్డులలో కూడా కొన్ని అత్యంత కీలకమైన రికార్డులను మాత్రం ఇవ్వకుండా దాచేశారు.
డైరెక్టర్ గారిని సంప్రదించిన వివరాలు:
ఈ విషయంపై మే 26, 2026 న నేను స్వయంగా విజయవాడ బ్రాంచ్ డైరెక్టర్లలో ఒకరైన మిస్టర్ సూర్య గారిని కలిసి మాట్లాడాను. ఆ రికార్డులు లేకపోతే—అవి ఎందుకు లేవో వివరిస్తూ హాస్పిటల్ అధికారిక లెటర్ హెడ్ (Letterhead) మీద రాసి ఇస్తామని ఆయన చెప్పారు. కానీ ఆ రోజు నుండి ఆయన నా ఫోన్ కాల్స్ ఎత్తడం లేదు మరియు నా వాట్సాప్ (WhatsApp) సందేశాలకు కూడా సమాధానం ఇవ్వకుండా తప్పించుకుంటున్నారు.

మీ హాస్పిటల్ వారు నాకు ఇచ్చిన ఫైల్‌లోనే కొన్ని నిబంధనల ఉల్లంఘనలు స్పష్టంగా కనిపిస���తున్నాయి. అప్పట్లో హాస్పిటల్‌లో అటెండెంట్‌గా ఉన్న నా తమ్మ���డు చంటి దారవత్ (Chanti Dharavathu) తో వారు చేయించుకున్న సంతకాలలో ఇవి గమనించవచ్చు:

1. అసంపూర్తి కన్సెంట్ ఫారాలు: కీలకమైన ఫారాలపై నా తమ్ముడి సంతకాలు తీసుకున్నారు కానీ, అందులో ఏ ప్రొసీజర్ చేస్తున్నారు, ఏ డాక్టర్ చేస్తున్నారనే వివరాలు పూర్తిగా ఖాళీగా వదిలేశారు. సుప్రీంకోర్టు మార్గదర్శకాల ప్రకారం ఇలాంటి ఖాళీ ఫారాలకు చట్టబద్ధత లేదు.
2. వేరే పేషెంట్ రికార్డులు కలవడం: నా కుమార్తె ఫైల్‌లో పూర్తి���ా వేరే పేషెంట్‌కు సంబంధించిన "High-Risk Informed Consent" ఫారమ్‌ను కలిపారు. నా కుమార్తె కేసుకు ఏమాత్రం సంబంధం లేని ఆ వేరే పేషెంట్ ఫారమ్‌పై నా తమ్ముడితో సాక్షిగా సంతకం ఎందుకు చేయించుకున్నారో అర్థం కావడం లేదు.

రికార్డులు లేకపోవడం వల్ల నా కుమార్తెకు చివరి రోజుల్లో ఎలాంటి చికిత్స అందించారో మాకు తెలియకుండా పోయింది. నేషనల్ మెడికల్ కమిషన్ (NMC) నిబంధనల ప్రకారం రోగి రికార్డులు పూర్తిగా పొందడం నా ప్రాథ���ిక హక్కు.
రెయిన్‌బో హాస్పిటల్స్ కార్పొరేట్ మేనేజ్‌మెంట్ మరియు చైర్మన్ డాక్టర్ రమేష్ కంచర్లా గారు దయచేసి ఈ విషయంలో జోక్యం చేసుకుని, నా కుమార్తె కేస్ షీట్‌లో మిస్సయిన ఆ నిర్దిష్ట రికార్డులను నాకు ఇప్పించాల్సిందిగా, మరియు తెలుగు మాట్లాడే ఒక ప్రతినిధితో నాకు ఫోన్ కాల్ మాట్లాడించాలని కోరుతున్నాను.
