I walked into the hospital, and a nurse directed me to the labor room to assist my colleague.
In the labor room, my colleague who had worked the night shift informed me that this was a case of prolonged labor, and the woman had declined a Caesarean section (CS).
I remembered her from her antenatal visits. We had discussed that the baby was too large based on scans and should be delivered via elective CS.
I asked her why she was declining the CS, and she told me to speak to her husband.
Her husband explained that she had given birth to large babies before and just needed time. This was her fifth pregnancy.
I called the on-call obstetrician to inform him of the situation. He arrived at the hospital within 10 minutes and requested that we discuss the matter with the family again.
The husband insisted that we were only after their money and that his wife was a strong woman who had successfully delivered four large babies.
Meanwhile, the midwife updated us that the baby was now in distress, but the family still declined the CS.
Eventually, after a few more hours and several manipulations, the baby was delivered but not breathing.
We attempted resuscitation with the on-call neonatologist for some time, but unfortunately, our efforts were unsuccessful. I was devastated.
To make matters worse, the woman bled heavily, necessitating a transfusion. She also sustained several lacerations that required repair.
At the end of the day, the husband refused to pay the medical bills because they lost the child. The CEO of the hospital eventually agreed to concede 50% of the bills before he paid.
Throughout the ordeal, all I could think was: this was preventable. But expressing that sentiment would not have been empathetic.
I couldnโt determine if their decision was truly based on financial concerns. The husband seemed average at least; he owned a car and had been paying all his bills.
Please, if you find yourself needing a CS, do everything in your power to ensure the safety of your baby. The nine-month experience of pregnancy should culminate in joy.

Myth 3: Lack of Pregnancy causes Fibroid.
Fact: This is simply not true. The exact cause of uterine fibroids is not fully understood, but several factors are believed to contribute to their development:
- Hormones: Oestrogen and progesterone, the hormones that regulate the menstrual cycle, appear to promote the growth of fibroids.
- Genetics: A family history of fibroids increases the likelihood of developing them.

Myth 4: All Fibroids are the same
Fact: Fibroids can vary widely in size, location, and type. They can grow inside the uterine wall (intramural), on the outer surface of the uterus (subserosal), or under the inner lining of the uterus (submucosal). The type and location of fibroids can affect the symptoms and treatment options.
