@CartersPeanuts@drkeithsiau I guess you missed my comment about butyrophenones/phenothiazines, which is essentially your point. Unless you’re on the Serengeti when you hear hoof beats think horses not zebras ie common things are common; rule them out first.
@drkeithsiau Drugs of abuse: Occasionally fentanyl can cause some dystonic reactions, tramadol can as well but it’s usually in patients on it and associated with rapid dose increase, promethazine (phenothiazine) is abused typically mixed with opioids/opiates. Tianeptine possibly.
@SteveCurryMD Given my initial training was heavily skewed to geriatrics, I was not a fan of two of these drugs. Both can cause hypoglycemia but since it asked “mostly likely”; I will go “all in” on the one that causes insulin secretion.
@SteveCurryMD 4 ANPP for fentanyl and levamisole for cocaine. Unless it was given lidocaine could be an adulterant in both. Likely has some ethanol on board.
