Cost of living for most parents.
A choice between giving a small family some decent standards of living.
Or a large family, facing factors like poverty, poor health care, minimal education and unemployment.
Wealth disparity needs to be addressed first.

They are citizens when they make these demands.
If everyone is given the right to practice their own culture, beliefs and faith you cannot mandate exclusion of certain cultures.

Yet to meet a doctor who will perform a major surgery from the goodness of his/her heart.

Maybe Optimus can finally bring some of that legendary nobility back into the medical profession .

Hundreds of millions of sick people in countries which provide no health care, complex surgeries or even doctors for people who cannot afford them.

Human doctors are not for everyone.
