Was an amazing @Pers_Care Holistic GP pop up clinic yesterday at the Wilds in the Riverside with partners @lbbdcouncil @thameslifecdt @MPSBarkDag @NELFT @barkingriver, Aurora Medcare and community groups/ charities - we saw around 150 residents @SP_LDN

Whilst the practice @nelft @lbbd was not funded we hope there will be funding for our community groups so we can carry this work via @thameslifecdt and we carry on innovating and building our neighbourhood team in @NHS_NELondon @MarieGNHS @DrGilluley @barkingriver

Rough sleeping is up by 9% this year and many of our guests have very poor #mentalhealth

Homelessness numbers are not likely to decline any time soon and pop up projects are providing an important service to the vulnerable.

In this Personalised outreach Homeless clinic - I never realised how much support we can provide for our community teams / caregivers and staff as well - the added benefit is that we can provide medical advise to them as well @Pers_Care @lbbdcouncil @thameslifecdt @NHS_NELondon

We saw about 100 homeless people in this PCN clinic and was able to provide a one stop service - true coproduction and the feedback was brilliant in this Homeless clinic and we picked up illnesses - this integrated approachtruly helped our most vulnerable residents @NHS_NELondon

In our community / GP outreach clinic love the food bank /welfare support /housings / social prescribing/our amazing community groups /@NELFT MH teams and Health visitors - give so much more than just seeing a Gp but also building that neighbourhood team at the point of contact
