SIR wrapped last week.
We debuted Wysdom 2.0 and hosted our first Wysdom event. Conversations with physicians and industry representatives on the floor gave us more product direction in three days than we could have planned for. The event had great energy, and the turnout exceeded our expectations.
To everyone who stopped by or came out: thank you. See you at SIR 2027 in LA.

@rustyhofmannMD keynote on physician-scientist entrepreneurship; built Grand Rounds & Wysdom, raising >$500M to tackle major healthcare problems.
Measure impact not by papers, but by patients reached. Physician-scientists extend their impact beyond 1000s of patients➡️millions.
We are thrilled to announce the joint winners of the Wysdom SIR 2026 Case Challenge Contest: @TheRealDoctorOs and Dr. Mikin V. Patel, MD, have won the Wysdom SIR 2026 Case Challenge! Both doctors practice at the Joint & Vascular Institute.
These two physicians collaborated to produce a full video on Plantar Fasciitis Embolization, demonstrating exactly the kind of peer-to-peer medical education that Wysdom was built for.
We look forward to seeing what they create next as they film with their Meta glasses. Congratulations to both.

We are proud to announce the first winner of the Wysdom SIR 2026 Case Challenge: Dr. James Duncan from @WashUMedMIR
His submission on the Central Venous Catheter technique was recognized for its clarity, structure, and the kind of feedback residents can genuinely learn from. It is exactly the standard we built Wysdom to enable.
We cannot wait to see what you film with your new Meta glasses. Congratulations, Dr. Duncan.
Two more winners will be announced later this week. Stay tuned.

Ben Horowitz: “Nobody was born a great manager. It’s a very unnatural job.”
“Startups get really hard when the product gets into market… When you’re building the product, it’s all good. How’s your startup? It’s doing fantastic - we’re building a product, it’s going to be great, it’s so genius, everybody I tell about it kisses me on the lips and says it’s wonderful. But then you get in the market and nobody wants it. Then it gets real hard, real fast.”
So the first job for any startup founder is build a great product.
“If you can’t build a great product, it doesn’t matter if you can build a great company.”
But once you’ve built a great product, your goal becomes building a great company. And to build a great company, you have to become a great manager, which Ben argues is a learned skill:
“Nobody was born a great manager… It’s a very unnatural job… But as CEO, as a manager, that’s what you have to do - you have to evaluate people’s performance. You have to correct them. You have to make sure they’re on task. And those kinds of motions, you have to learn how to do them, and you have to learn how to do them in a way that everybody doesn’t hate you all the time.”

Peter Thiel on the question he asks every startup founder he invests in
“Why will the 20th talented person join your company when they can get paid way more at Google, they will have to work way less hard at Google, and it will look better on their resume to go to Google?”
The 20th employee won’t get the equity or prestige that someone on the founding team will get, so there needs to be another incentive if you’re going to build a truly great company and attract the best people in the world.
Thiel believes the best answer is something along the lines of:
“This is the only place in the world where you can work on this incredibly important problem.”
He continues:
“It has to—on some dimension—be a really important problem that at least some people think is the most important problem in the world. Those are the kind of businesses that are unique, and when they work, they end up being leaders in their respective markets.”
If it’s a problem that a bunch of other team are working on, it will be hard to attract a truly world-class employee #20.

For 25 years at Stanford we have stented below the inguinal ligament and advocated as such in appropriate patients. And after every talk I give, people ask about safety etc. Tomorrow will present data on why you need to do it, when the vein isn’t normal.
Don't forget! Register https://t.co/4jnBHg3Y4c for tomorrow's virtual webinar (7 pm EST) "To stent or not to stent below the Inguinal Ligament, that is the question" w/ Drs @rustyhofmannMD, @DrSteveElias, Sheila Blumberg, Tony Gasparis & Nicos Labropoulos.

In Barcelona at Paradiso (one of the top 10 bars in the world) with colleagues at @cirsesociety Bonus points if you can name more than 5 people! @CVIR_Journal@JVIRmedia. Great representation from past and present @Stanford_IR trainees!
