Basic checklist of questions I ask candidates on initial phone screens (changes based on opportunity):
Biggest impact?
Direct report titles?
Title reports to?
Peers titles?
Team size?
Rev/P&L size?
Why interest in role?
Open to relo?
Resume?
Comp expectations?

Any executives out/looking - go to executive search firms' websites and find the practice leader in your vertical. Most list their emails. Send resume and brief background in the email. It will get around.
