So you need to learn about medical billing. Each item on that bill has a cpt code. Each code has a max Medicare allowable. All providers must bill all Ins companies the same amt so bill 2-3 times that amt. What the ins doesn't pay per their contract is adjusted off. If you divide by 3 you will get the true bill.

@clerpatriot Providers don't set the rates, Medicare does. Any thing billed is a multiple of a rate set by Medicare for each CPT code. INS companies are paid on a set amount by contract regardless of what a provider charges.
