A 62-year-old woman experienced recurrent fainting episodes, double vision, eyelid drooping, and weakness. Antiarrhythmics monitoring: Sotalol/Dofetilide (no CKD): Watch QTc, electrolytes, torsades. Flecainide/Propafenone (no structural HD): Stress EKG for QRS widening, add rate control to avoid 1:1 flutter. Amiodarone: PFTs/LFTs/TFTs for toxicity. Success varies case-by-case.
