10 years ago my wife, the mum of our kids & the MP for Batley&Spen was killed by a far right extremist.
At anniversaries I try to be optimistic about the future. But not this time. In the ten years since she was killed we have gone backwards & I fear our democracy is now at risk
Near-miss reporting rates are one of the most powerful predictors of site safety.
High reporting = high awareness = strong culture.
Low reporting = workers don't feel safe speaking up.
If your site has low near-miss reports - don't celebrate.
Investigate why that is.
Before any high-risk task, ask your team 3 questions:
1. What could go wrong today that didn't go wrong yesterday?
2. What are we most likely to skip when we're under pressure?
3. If someone new joined us right now, what wouldn't they know?
Those answers are your real risk assessment.
The paperwork bit comes after the thinking.
6:00am was the safety briefing.
A contractor didn’t sleep.
Arrived late.
Up fighting with his wife most of the night.
He was fuming.
Grabbed his tools.
Then rushed on to the site.
Was not in the right mindset.
This is when your safety system either works or it doesn’t.

Might be a technical psychology term... but very easy to get your head around.

Once you see the gap between how work “should” happen and how it “actually” happens, you can't unsee it.
