@TBach4 @ClevelandClinic Was not told it is a separate charge. If I felt this was issue I would have seen a psychiatrist. Follow-up for blood pressure appt has nothing to do with this. They said by law the have to ask these questions. Unreal. I refused to pay for it and the sent it to collections.

@TBach4 @ClevelandClinic Same thing happened to me with @clevelandclinic. Normal follow up visit with doctor and they had pre appointment questions online they asked me to fill out. I started answering the questions and stopped halfway because it had nothing to do with my appointment.
