Patients represent a real-world population treated at a single US medical center during the study. The median peak ALC was 3.2 × 103/μL. Across the full cohort, 16 patients (30.2%) developed peak ALC expansion >5 × 103/μL. For patients in the preintervention group (received cilta-cel between September 2023 and July 2024), 9 of 30 (30.0%) developed a peak ALC >5 × 103/μL; 5 of 9 (55.6%) experienced atypical neurologic events, and all 5 patients died of complications related to cilta-cel treatment. In the intervention group (patients who received cilta-cel between August 2024 and January 2025), 7 of 23 patients (30.4%) developed a peak ALC >5 × 103/μL, all of whom received ALC-directed dexamethasone prophylaxis. Only 1 of 7 of these patients experienced an atypical neurologic event, and the only death in this cohort was due to an infectious complication 9 months after CAR T-cell treatment.
