Between February 2023 and June 2023 kidney doctors at Aster MIMS Hospital, Kottakkal, in Kerala started getting a series of patients with a rare and strange type of kidney disease. They were baffled.
They called it Neural epidermal growth factor-like 1 protein (NELL-1) associated membranous nephropathy - a weird disease where the the body's immune system was attacking the filtering membranes of the kidney.
The disease was characterized by massive loss of protein in urine (>3.5 g/day) and clinically presented with severe leg and face swelling, high blood pressure, frothy urine, and clot formation in blood vessels of legs or lungs and sometimes strokes.
The first patient was a 14 year old girl, followed by her relative. Both their kidney biopsies were positive for NELL-1 kidney disease.
Then came a completely unrelated 29 year old man. The doctors were alarmed. And they looked at their records. There were five more such patients within the past 4 months.
They looked at published literature and found a common culprit among most NELL-1 cases recorded.
Mercury.
Blood levels of mercury in patients were checked and the doctors were stunned. Levels were 100 times more than normal limits of detection.
But what was common to all these mercury poisoned unrelated patients?
And a thorough clinical history in retrospect found the culprit.
Fairness creams.
All patients were using locally made fairness creams to lighten their "naturally-beautiful dark skin."
Fairness creams contaminated with mercury, causing chronic mercury poisoning and NELL-1 kidney disease is well known.

Another cause for Nell-1 kidney damage is presence of lipoic acid which can by itself cause membranous nephropathy. Lipoic acid is a common constituent of fairness creams and many oral antioxidant supplements - be wary of this.

Not only fairness creams, but traditional herbal medicines, especially Ayurveda & Siddha formulations which Indian "wellness gurus" such as Sadhguru so passionately promote, have been implicated in multiple cases of mercury-induced severe Nell-1 kidney disease.

Be wary of locally made cosmetics, lipoic acid based oral supplements and Ayurvedic and Siddha herbal products. Know what is good for you. And more importantly, know what is not.
Avoid unwanted troubles. Use the knowledge from medical science as a shield to defend yourself against unscientific practices and bad science.

Our paper on Ashwagandha-induced liver injury from India just published in @HepCommJournal
Our paper is important in three aspects:
1. It is the largest series of Ashwagandha-liver injury in current medical literature
2. It is the first series from Asia-Pacific region
3. It also reviews the benefits of Ashwagandha as per highest quality of evidence in various conditions and proves that "no good clinical levels of evidence" exist to support the use of this herb, contrary to what the social media health influencers, nutritionists and fitness experts say.
➡️Here are salient features of our study
✅Patients enrolled from 3 centers in India - we ruled out ALL OTHER causes for severe liver injury in these patients as per standard protocol for diagnosis of herb-induced liver injury
✅Commonest reason for Ashwagandha use was for anxiety/ stress reduction and sleep
✅Three patients had pre-existing liver disease - Ashwagandha liver in them was severe, all three died without a timely liver transplant
✅In those without underlying liver disease, even though clinical course was prolonged, all recovered
✅But one patient developed chronic herb-induced liver injury - revealing the fact that herbal liver injuries can lead to development of cirrhosis in short term unlike other causes of cirrhosis
✅Jaudice and severe itching were most common symptoms
✅Chemical analysis of retrieved herbal formulations did not reveal other adulterants or contaminants
✅Including ours, there are 22 cases of Ashwagandha liver injury published globally - we do not know how many diagnoses are missed or unreported [See Table 3 in the paper]
✅We looked at clinical levels of evidence for all indications published on Ashwagandha and found that none were good enough to recommend this herb, studies lacked rigorous methodology, had inclusion bias, poor follow up, weak surrogate measures and only short term analysis [see Supplementary Table S1 online and also Figure in the next tweet] @DrArunKValsan
✅Those who promote Ashwagandha use on social media and on their respective websites, based on weak levels of evidence, do so, because they have vested interest in selling this as a supplement and not otherwise.
