The team led by paediatric reg acting up as consultant identified cardiac failure secondary to a congenital cardiac condition likely a VSD.
High flow to support breathing, antibiotics and frusemide bolus was initiated.
A second assessment was made which led to identification of a large liver and consideration of cardiac failure.
Further history from mother - identified sweating with feeding and not gained birth weight.
following initial assessment gave facial mask oxygen and gave a 10mls/kg bolus following a blood gas that showed mixed respiratory and metabolic acidosis. ( lactate 3.4 and normal electrolytes and sugar)
With the bolus the baby dropped saturations and work of breathing increased.
Initial assessment
A- unsettled crying
B - use of accessory muscles, recessions and crackles bilateral, saturations 93-94%
C- CRT- 3-4 sec, HR- 170,
systolic murmur, pulses easily palpable, large liver
D- normal blood sugar
E- afebrile and normal pupillary response
Scenario -3 week old poor weight gain presenting with increase work of breathing and poor oral intake.
ED team and Paedatric team participated.
Objective: Undertake ABCDE assessment, identify the underlying cause for the symptoms & initiate management based on clinical findings.
