@theliverdoc Sleep MD here…great post. Melatonin as a supplement is not regulated by FDA so we get patients taking 20mg +QHS. Soporific for some, but by in large useless for insomnia. Gold standard treatment is Cognitive Behavioral Therapy for Insomnia.
@DGlaucomflecken I think it depends on the student. No need for someone going into pathology to do more that a night or two of float IM or OB. Students going into fields that have overnights can do them during their Sub-Is and show rotations.
@AnthemBCBS Oh I already had a P2P my “peer” told me my note was too long. So basically you have employed people who are making medical decisions affecting patients lives without reading physician notes.
So @AnthemBCBS denied an in lab psg for patient with HFrEF, EF 30%, BMI > 40 and bicarb of 33. Just a reminder that physicians who work for insurance have no concern for patients.

It is simple math that we cannot reduce healthcare costs in this country without reducing the amounts we pay doctors and hospitals.
Dr's cannot make $30k/hour in an affordable system.
If humans cut themselves off from all zeitgebers and follow their natural rhythm they would free run akin to Non24. Humans biological rhythm is around 24.15 hours give or take. This is not the reason that most people have trouble sleeping. There is no biohacking to be done here.
