When the temporomandibular joint (TMJ) is held in an extreme, prolonged open position for hours a day over decades, the capsular and collateral ligaments undergo structural changes. Ligaments are dense bands of connective tissue designed to stabilize joints; however, unlike muscles, they have very limited elasticity. When subjected to continuous, long-term hyperextension, they can suffer from permanent ligamentous laxity (elongation). Once these ligaments are stretched past their anatomical limit, they cannot naturally "snap back" into place on their own. An unstable joint moves abnormally, which can prematurely wear down the protective cartilage and bone, leading to localized osteoarthritis.
