What may I do in a general OP PT clinic with a guy, 5 months post GBS onset. -4/5 BUE MMT and 2# grip strength on RUE. -2/5 gross BLE MMT, besides seated hip ABD 4/5. Fully dependent.

Clinic does not have safe transfer equipment to mat table.

In rural wash state.

Started Contract #6 last week in Washington State! Currently slated for 13 weeks. Found a unicorn clinic.

- 1 on 1 care for 45 minutes
- 45 min eval with 15 minutes of documentation time afterwards.
- paid 15 minutes of pre-charting to start the day

@Andy_Cyburt15 Idek I met them the first week I was there. It was even worse...

For example. A medicare/FFP would have a referral for knee pain and they would he treating shoulder pain.

A referral for neck pain and they would treat just gait and balance therx.

Traveler Scenario:

Took over a patient from post-op about 10 weeks, about 8 weeks in physical therapy from the previous PT. They were seeing by the previous traveler at the clinic who embraced the 3x a week with government based insurance with a hard cap of 30 visits per year.

Part 3:

After denied by government insurance for auth again due to patient should complete therx program via HEP, patient confronts me saying I did not do my job of getting visits approved.

Part 2:

When I took over POC, I discussed only having x amount of visits left and need to space them out. Patient still wanted 2x-3x a week and we soon ran out after 4 weeks.

Fast forward...patient hit 30 visit cap, and been trying to insurance auth ever since.

Also cool, living this travel contract life, housing and auto account for over 65% of my total expenses for 2024.

2024 was the first full year of working a full-time job and first of PT career. Also, first year of tracking every expense! Exciting insights on how much you actually spend and on what.

So far:
Week 1: 40 hours (ramping off/75% caseload)
Week 2: 18 hours work + 3 days of vacation
Week 3 & 4: 28 hours work + holiday off + 2 half days

Gotta love Healthcare travel contract work
