👉 8/10 people develop spine problems at some point.

The real confusion isn’t the pain—it’s who to consult: GP, physio, ortho, neuro, neurosurgeon, spine specialist?

Here’s the simple truth 🧵👇

Most back pain is benign and improves with time, exercise & guidance.
But in some—especially older adults—it may signal fracture, infection or cancer. Early diagnosis matters.

❌ Not everyone needs an MRI.
MRI is usually needed only when there is:
• Weakness/numbness
• Bladder/bowel issues
• Functionally disabling pain
• Pain >3 months despite treatment

🦴 MRI shows nerves & discs.
📸 Standing X-rays show spinal instability.
Both have a role—when used wisely.

Spine surgery is based on just 2 principles:
1️⃣ Decompress compressed nerves
2️⃣ Stabilise unstable bones

Technology has evolved → MIS → navigation → robotic spine surgery (think maps → GPS → live GPS).

Who offers the best care?
Dedicated spine specialists (ortho/neuro background) focus only on the spine and offer stepwise care—from education to injections to surgery only when needed.

Bottom line:

Young doctors are avoiding surgery — “too stressful, too long, too demanding.”

After 30 yrs in medicine & 25 yrs as a surgeon, here’s my take:

Surgeons cure. Physicians control.
If it’s structural — tumour, fracture, disc, obstruction — a surgeon gives a definitive solution.
If it’s chemical — it’s managed, not cured.

Yes, surgery is tough.
Yes, it costs years of your life.
But it gives you the power to change someone else’s life in a single intervention.

If I had to choose again,
I’d still choose surgery. Every time.

Students:
Don’t choose the “easy” path.
Choose the one that lets you deliver a cure.
