My wife (Ria) went to AIIMS. I went to IIT Delhi.

We watched 'Hello Bachhon' on Netflix this weekend. One kid in the show breaks down walls in his village to sell bricks so he can afford coaching fees. A girl hides her textbooks because her parents want her married off.

Throughout the show, Ria and I realized that we never had to fight for any of that.

We had access to coaching. Good coaching. Neither of us ever worried about whether we'd get to prepare.

The IIT-AIIMS crowd loves talking about the grind. # of hours of self-study per day, the pressure, and sleepless nights. I've done it too. But that grind starts after you already have access. Most of us skipped the actual hard part and don't even know it because the people who are capable but can't afford access are invisible. They're not in your coaching batch. They're not in your college. You never compete against them because they never got the chance to show up.

We credit hard work more than it deserves. Sometimes the biggest advantage was just being born in the right city to the right family.

'Hello Bachhon' made this truth hard to ignore. Great watch.

अतुल सौरभ की आत्महत्या पर

अतुल की आत्महत्या के बाद सोशल मीडिया में जैसे चर्चा हो रही है, हम सब को लगता है कि इससे परिवर्तन हो जाएगा। मुझे ऐसा नहीं लगता।

अतुल सौरभ जैसे दर्जनों केस हर वर्ष होते रहते हैं। अतुल एक सुलझे हुए व्यक्ति थे जिन्होंने वीडियो और पत्र के माध्यम से अपनी पीड़ा और आत्महत्या के नि���्णय पर प्रकाश डाला। हर पीड़ित के पास अभिव्यक्ति के साधन नहीं होते। वो बस तंत्र से हार कर मर जाते हैं।

तंत्र से जीतना असंभव है। अतुल की मृत्यु से कुछ नहीं बदलेगा। मैं आश्वस्त हूँ कि वही महिला किसी और से विवाह कर के, अपने बेटे को ग्रूम करते हुए, बाप के बारे में विषैली बातें करते हुए उसे पालेगी। संभवतः कोर्ट उसे अतुल की संपत्ति का पूरा हिस्सा भी दे देगी।

जहाँ तक जज की बात है, जजों को जज बचा लेते हैं। सुप्रीम कोर्ट के मुख्य न्यायाधीश से ले कर जिला न्यायालयों के परिसर में धूल फाँकते वकील को भी पता है कि कोर्ट हर स्तर पर घुसखोरी का अड्डा है। तो, इस पर भी कोई चर्चा तक नहीं होगी।

परसों तक यह विषय समाप्त हो जाएगा और हमारे पास नया विषय आ जाएगा। यही सत्य है।

मैं अतुल से सहानुभूति भी रख कर क्या करूँगा? वो तो चले गए। अतुल को मरने से पहले एक-दो को साथ ले कर जाना चाहिए था। मुझे ‘शूल’ फिल्म का अंतिम दृश्य याद आता है जहाँ मनोज बाजपेयी के पात्र को तंत्र ने विवश कर दिया कि वो विधानसभा में विधायक की कनपटी पर बंदूक ��ख दे।

समाज ऐसा ही न्याय चाहता है। संभवतः न्याय है भी वही। परंतु, कानून की पुस्तक में वो न्याय नहीं हत्या हो जाएगी। कानून की पुस्तक में अतुल ने आत्महत्या की है, पर समाज जानता है कि अतुल की हत्या की गई है।

न्यायपालिका के हाथ कानून से बँधे हो सकते हैं, पर जो पेशकार जज के नीचे बैठ कर पैसा लेता हो, और जो जज स्पष्ट रूप पाँच लाख माँग रही हो, उसे राह चलते कल को कोई गोली मार दे, तो वह अन्याय नहीं होगा। क्योंकि उस जज ने केवल घूस नहीं माँगा, उसने उस व्यक्ति की हत्या में अपना योगदान दिया है।

कई लोग कह रहे हैं कि जज को पैसे दे कर सेटलमेंट कर लेता, जीवन क्यों ले लिया अपना। यह बात आप इसलिए नहीं समझ सकते क्योंकि कुछ लोग अपने आदर्शों से इस सीमा तक समझौता नहीं कर पाते। पेशकार को सौ रुपए देना भी घूस ही है, पर पेशकार से न्याय की आशा नहीं है।

वहीं, कोई जज हर बात जानते हुए भी आपको विवश करने लगे, तो आप ट्रिगर हो जाते हैं कि ये कब तक चलेगा? भगत सिंह चाहते तो असेंबली से भाग सकते थे बम फेंकने के बाद। वो नहीं भागे क्योंकि उन्हें लगा कि उनकी फाँसी से आंदोलन को बल मिलेगा।

अतुल की आत्महत्या भगत सिंह के स्तर की नहीं है, पर सोच वही है कि उसकी मृत्यु से कोई परिवर्तन हो, कोई नोट करे कि यह अपवाद नहीं, उदाहरण है। अतुल को जब लगा कि पूरा सिस्टम, न केवल उसे बल्कि उसके पूरे परिवार को टा��्गेट करता रहेगा, उनके दिए पैसे ही उनकी पीड़ा का कारण बनेंगे तो उन्होंने यह कठिन निर्णय लिया।

हम स्वयं को ले कर जितनी चिंता नहीं करते, उससे अधिक अपने माँ-बाप या पत्नी-बच्चे को ले कर चिंतित रहते हैं। यदि वैसी परिस्थिति आती है, तो हम अपने लिए नहीं, उनकी पीड़ा को ध्यान में रख कर कॉम्प्रोमाइज करते हैं।

अतुल की आत्महत्या उसके परिजनों के लिए कॉम्प्रोमाइज है। उसे लगा कि वह यदि इस समीकरण से हट जाता ह���, तो बाकी हर व्यक्ति चैन से रह लेंगे। पुत्र का चेहरा भी उसे याद नहीं था, वह छोटा है, बड़ा हो कर उसे कदाचित् पिता का पक्ष समझ में आ जाए, ऐसा अतुल ने सोचा।

पुलिस, न्यायपालिका आदि जितनी भ्रष्ट हो सकती है, हो जाएगी। इनके लिए अतुल उन सैकड़ों नामों में से एक नाम है, जिसे ये हर सप्ताह वसूली का वॉलेट समझते हैं। उन तक ये वीडियो पहुँचा भी होगा तो ‘चू#% साला’ बोल कर समोसे खा लेंगे।

यही सत्य है। यही दुर्भाग्य है।

Woke up to the news that a patient of mine died at a nearby hospital from a bacterial infection that responded to zero antibiotics. Zero. The bacteria, Acinetobacter baumanii was resistant to every single antibiotic tested. Every one of them. He was only 62 years old. He had an easy 10 years left, if not for that infection.

Individuals, doctors and specific communities are to be blamed for such deaths. Not the liver disease. Everyone of us, through actions and silence are to blame.

India is rapidly becoming the hub of multi-drug resistant bacterial infections in the world.

There is a lot we can do at individual and group level for this nightmare to dim out.

1⃣Please stop taking antibiotics on your own, especially for a fever, cough/sore throat which in the majority is caused by a virus, not a bacteria.

2⃣Please stop stockpiling antibiotics at home to use at your own whims and fancies.

3⃣Once started for a bacterial infection, please do not stop the course mid-way and complete the course as advised. Most antibiotic courses range from 5 to 7 days and sometimes more in case of complicated infections.

4⃣Please do not take antibiotics based on "pharmacist prescriptions" over the counter.

5⃣Pharmacists, please do not 'suggest' any antibiotics over the counter because "it is a popular choice" based on doctors prescriptions. You are not 'doctors' to suggest any medicine anytime. So dispense and educate the public on what needs to be done and the needful adverse events of medications (which most doctors fail to discuss on) that they need to know of.

6⃣Doctors, please do not prescribe antibiotics as if fever is a symptom of antibiotic-deficiency. Every patient you meet have not come to you for an antibiotic prescription.

7⃣Doctors, please do not prescribe antibiotics over social media such as Whatsapp without seeing or examining patients to confirm bacterial origin of infection.

8⃣Doctors, please do not start an antibiotic the moment a patient enters the hospital emergency or in-patient department. Not all patients are admitted for infection control. Prescribe antibiotics when reports/cultures for the same or the clinical situation (immunosuppressed) warrants it.

9⃣Doctors, please do not upgrade antibiotics (eg: directly to meropenem from ceftriaxone) because the fever did not go away in 8 hours time and do not keep upgrading and changing antibiotics every day because the fever spikes keep coming, even though intensity and frequency are reducing. Medicine is not magic.

🔟Pseudoscience peddlers - Homeopathic and Ayurveda practitioners and Naturopaths, please dont even think about prescribing antibiotics, because contrary to what you believe and what the Government has empowered you to believe, you are not doctors and are not competent in treating anyone, including single-celled organisms. Turn around, face the wall and stay there. And stop adulterating your junk remedies and supplements with antibiotics.

⏸️Prescription of antibiotics is the singular litmus test for a doctor, that takes into consideration EVERYTHING that makes that person a "doctor" - it tests the physician's knowledge, skill, reasoning, critical thinking, morality, ethics and humanism. You fail at this, you fail in your duty. Be a better human first, being a good doctor will follow.
