Not all shoulder pain comes from the shoulder.
These are referral maps. Pain arising in the cervical spine and the periscapular soft tissues does not stay where it starts. It refers across the trapezius, over the scapula, and in some patterns onto the anterior chest wall.
Two practical consequences.
A patient pointing to the top of the shoulder is not necessarily describing a rotator cuff. Cervical and myofascial sources produce a very similar map, and the examination has to separate them before imaging is asked to.
And a normal shoulder MRI in a patient with convincing shoulder pain is not a dead end. It is a prompt to examine the neck.
If the pain does not change with shoulder movement, stop moving the shoulder.

Hamstring cramping or tightness and back pain can be early warning signs that someone is at risk of developing a hamstring muscle injury.ย
If the athlete is reporting these we may need to suggest a reduction in high speed running and a focus on recovery, especially if they are recovery from injury or reporting fatigue.
