Letter from Calgary group (RAPM, 2019) already stated PENG should be done pre-surgical to avoid quads weakness (prior to fascia disruption). Also, why avoid skin infiltration in the PENG group?, unless also planning on doing LFCN.

I'm hopeful it can help but concerned about brachial plexus involvment after c-spine surgery confusing the picture. Also risk of bilateral phrenic nerve involvement. Any thoughts/advice

We used 20cc of 0.25% bupi. More volume or concentration can cause proximal tracking along psoas with motor nerve involvement. Also, inadvertent injection within the psoas muscle can track to the femoral nerve.
