Last night in Washington DC, United States 🇺🇸 was a great one for the Governor of Lagos State, Mr. Babajide Sanwo-Olu as he received the 2026 Global Transformation and Good Governance Prize by the Millennium Excellence Award, USA. In his speech after the award ceremony, Mr. Sanwoolu-Olu, who dedicated the award to the great people of Lagos, declared that the future of African investment will be determined not just at the continental or national level, but in cities where infrastructure, businesses, and economic opportunities are created.

Ending AIDS is not possible through biomedical interventions alone. The evidence is clear that stigma, discrimination, punitive laws, gender inequality and violence can prevent people from accessing HIV prevention, testing, treatment and care. The UN has explicitly recognised these structural barriers as undermining progress towards the 2030 goal.

The new Political Declaration is therefore important in reaffirming human rights, gender equality and the removal of barriers to services. This is not an ideological add-on to the HIV response. It is epidemiology and public health in practice.

We have the science to prevent HIV, treat it effectively and achieve U=U. Now we must ensure that people can actually access those tools without fear, discrimination or exclusion.

To end AIDS by 2030, we must treat the virus and address the inequalities that allow it to spread. Human rights are not separate from the HIV response. They are part of the response.

Community-led services are essential infrastructure of the HIV response, not an optional complement. They are often the bridge to people who face stigma, discrimination or legal and social barriers that can keep them away from conventional health services. UNAIDS notes that community organizations can reach up to 60% of marginalized populations in some settings.

The financing situation is therefore deeply concerning. International HIV financing fell 18% in 2025, while UNAIDS reports major reductions in community-led services, including an 85% reduction in services for gay men and other men who have sex with men and 82% for sex workers across a recent 47-country study.

As countries transition towards greater domestic ownership, we must protect and institutionalize community-led delivery through sustainable domestic financing, social contracting and community-led monitoring, while maintaining international solidarity during the transition.

If we defund the communities that reach those furthest from care, we risk losing the very people we must reach to end AIDS.

It was quite a success, as the scheduled robotics-assisted surgery was duly carried out at the Robotics Center of the Imo Specialist Hospital, Umuguma. I am pleased to report that the patient is stable, recovering quickly, and responding well to the procedure.
