That’s a porcelain gallbladder — extensive calcification of the gallbladder wall, usually associated with chronic inflammation and gallstone disease.
The important malignancy to exclude is gallbladder carcinoma. Interestingly, newer evidence suggests the cancer risk is lower and more variable than historically reported, depending on the pattern of calcification.
