Progress/palliation in esophageal cancer — thoughtful integration of endoscopic therapy with med/surg oncology.
Grateful for my training with @neilRsharmaMD and excited to bring similar approaches to our patients at @MyFranciscan

@JAMASurgery Minimizing percutaneous drainage time should be a top priority!
Internal endoscopic drainage is consistently shown to be superior and is a large gap in surgical education/training.
Perc drainage is not benign!
@JAMASurgery@Dr_Chris_Brown Perc CCY tubes, in general, should be avoided. High morbidity, patients despise them, and endoscopic approaches that are widely available provide durable internal drainage and allow future surgery. #gitwitter # surgery
@SChandanMD Thanks Saurabh! I prefer overstitch for full thickness bites. This was a standard duodenoscope - TJF-Q190V. I’ve gotten into the habit of trying to place the LAMS in the proximal/mid body and dilating the pylorus as needed to ease scope passage.
One-stage EDGE performed for a RNY pt with symptomatic choledocholithiasis.
Transgastric access obtained w/ LAMS → stent fixation → ERCP with duct clearance.
A reliable option for RNY anatomy that avoids the long-limb ERCP challenges in altered anatomy. #GITwitter#ERCP#EUS
@GaiLonghurst@drkeithsiau You’re right, but used figuratively, ‘acquisitiveness’ can mean a strong desire to learn or increased curiosity, though less common and depends on context. I’ll leave it to ur interpretation. ‘Inquisitiveness’ might be a better fit, but it didnt flow as well with the ‘A’ theme ha
EUS-guided cholecystoduodenostomy for acute cholecystitis in a non-surgical candidate.
LAMS deployed, pus drained.
Always satisfying to see the immediate relief these pts experience.
Minimally invasive. Maximally impactful.

Thank you, @neilRsharmaMD — truly means a lot. I’ve been lucky to have mentors like you who emphasized not just procedural/clinical skills, but also the value of team building, multidisciplinary care, & building trust across specialties. Excited to keep growing and contributing.
Amazing work here!
Great to see the growth of the field interventional Endsocopy by amazing physicians who go out and build programs at tertiary community hospitals !
This takes clinical and leadership skills

@muhammadbaigmd Thank you! Patient had a slightly torturous CBD requiring EUS to be in a relatively unstable position to get good access. Patient also had partial GOO, want to reserve ability for future stenting if EUS-GE cannot be done.
