It ignores the fact that the price wholesalers pay to manufacturers determine the cash out of pocket to pharmacies. Nor does it mention the float the pharmacy carries for the PBM
If a branded med is $600 minus 5pct discount , that's 570 the pharmacy pays in cash.
If the copay is 20. That's cash out of pocket of $550 to the pharmacy , plus the cost of float while it's in inventory.
Add point of sale rebates to patients and the pharmacy is the bank for even longer.
Change the price brands charge wholesalers to net and everything changes.
They didn't say a word about this.
They can't be that clueless. They must have wholesalers and PBMs as clients .
The big insurance companies and their PBMs and the wholesalers look for the weakest link in the supply chain, independent pharmacies , sponsors, patients and where applicable, independent physicians and underpay, slow pay, audit and clawback and look for any way to generate fees.
Someone explain to every patient who is paying too much for brands , why it works this way. It makes no sense to me
If brands sold to wholesalers at net price , they would make more money.
If wholesalers bought at net, they could markup the drugs they sell to pharmacies to match the Prompt pay and Data refunds they get, and earn a great cash on cash return
The losers ? The big insurance companies and their PBMs. That's how much I checked power they have !
