Go medial and take more of the facet
Make room lateral by drilling some of the rib head
Ream through to create room (different TYPES of reamers also)
Sanjay Konakondla, MD (Danville, PA), discusses the challenges of identifying the dura when entering the epidural space with an endoscope. Dr. Konakondla emphasizes that peridural membranes are real and intradural fat is not a normal finding. If fat is visualized between tissue layers, it indicates that the tissue is not dura. After adequate coagulation, these layers can be carefully dissected and safely removed.
