@OGdukeneurosurg Cherry red skin isn’t quite pathognomonic for CO toxicity. It can be seen with anything that results in high venous O2. Electron transport inhibitors like cyanide, hydrogen sulfide, azide for example could also cause it.

@KeithNHumphreys Although multiple doses are often given, I question how often it’s actually necessary. Often times a second dose is given pretty quickly before the onset of action of the initial dose.
@hspungenmd@SteveCurryMD The bicarb can be inferred by looking at the pH and the PCO2. Is the PH as high as you think it should be based on how low that PCO2 is?
@db0255@armyemdoc@joshmcgoo The body does have an endogenous opioid system. However, if one is naive to exogenous opioids (and therefore not physically dependent), a dose of naloxone will not result in any untoward effects.

@armyemdoc@joshmcgoo That’s the point. You can’t tell the difference. So, it is wise to start with a low dose and go from there. Why blast them with 4mg and potentially cause severe florid precipitated withdrawal, which isn’t without harmful potential complications?

Leads to inappropriately assigning causality and inappropriately r/o causality. Also potential for stigmatizing the pt.
