Thanks to @wesstreeting for looking into some possible perks for Dr employment, such as:
Not being unemployed!
Absolutely mind-blowing perk for a job, actually having a job 😍
Wes Streeting’s leaked offer letter to the BMA confirms what many of us feared: there is still no serious intent from him to resolve this dispute.
🔴 No offer on pay - despite this being a pay dispute.
🔴 Vague references to “exploring” help with the cost of equipment, meals, and accommodation - basic working conditions, not a substitute for pay restoration.
🔴 His commitments from last year such as rotational training and exception reporting reform still haven’t even been delivered.
This isn’t meaningful negotiation. It’s political deflection.
Doctors are not asking for NHS meal deals or vague promises. We are asking for a credible offer that begins to reverse a 21% real terms pay cut.
That means moving a fully qualified doctor from £18.62/hr to £22.67/hr, over time, not overnight.
If the Government is serious about ending this dispute, it must stop recycling old promises and start addressing the core issue: pay.
If this was me, I would buddy up with the other 260 people and take legal action and seek compensation for distress and disruption to career progression
The MRCP exam debacle surely must be the final nail in the coffin. These ‘Royal Colleges’ are disgustingly draconian, out-of-date, and inadequate when it comes to Postgraduate medical training.
They must be revamped/abolished and medical education fixed in this country.

Are you intending to submit evidence to the Leng Review?
I haven't seen how to do so shared widely by DHSC or NHSE.
It's via this email:

PLEASE SUBMIT YOUR CONTRIBUTION ASAP.

The obvious thing to state is that this is clearly a lie and a ‘reduced involvement’ timetable has been created to prevent a backlash as to the extent of what the PAs in the Trust are actually doing
Update: @ImperialNHS have now published details of their Physician Associates in paediatric surgery
Worryingly, the role includes assisting with operations on children, including assisting with emergency surgical cases 🚨
Are parents informed as part of the consent process?
PA goes to medical school:
Realises they THOUGHT they knew everything but medical school taught them how much they didn't know.
Respect to this PA for going to medical school.
Am I surprised they were humbled by medical school? No. You don't know what you don't know.
The Irish Medical Council (GMC equivalent in Ireland) are taking a stand!
They state:
“As the independent body which regulates doctors in Ireland, they are NOT the appropriate regulator for physician associates”
The 4 reasons they give below:
🧵
This argument under a bus shelter in Bradford is I think the greatest insight into the White English Mind. Capital punishment, EU law, Ruth Ellis, shipyards, royalty, WW2, beheadings on the streets of London. It's all there.
@gmcuk So, you're going back to the drawing board, right?
Based on less than 30% overall agreement from patients/public and an abysmal 13% agreement from >1000 medical pracitioners, one could only hope this would prompt you to reflect and revise.
Who will mark your homework?
The GMC is effectively saying that foundation program, core training, specialty training, exams, portfolio, ARCP, curriculum, 48 hour weeks, audit - all the hoops and jumps we go through are entirely superfluous to practice medicine. All it requires is local, unregulated, non
@gmcuk is not fit for purpose & it is negligent for @wesstreeting & @LaylaMoran to continue to fail to take appropriate action including appointing an interim CEO & announcing a rapid turn around enquiry. 1/8
Not to add that they happen to only publish this 200+ page report hours before the debate in parliament. So so sneaky and so so disingenuous.
Disgrace of a regulator.
The ‘key changes’ they’re proposing are nothing short of an abdication of responsibility and accountability.
The experiment has failed.
By saying that most of the negative feedback received was outside the scope of consultation’, you make the consultation pointless.
Shame.
Today, we’ve published a report on the outcome of our public consultation on the proposed rules, standards and guidance that we’ll use to implement the regulation of physician associates (PAs) and anaesthesia associates (AAs).
The report:
- details the key changes we have made as a result of the feedback received
- provides a comprehensive breakdown of all responses
- addresses a range of broader issues raised during the consultation.
We’ve also shared our findings with relevant bodies across the system to encourage them to act on feedback that relates to their areas of responsibility.
We’re grateful to the over 3,000 individuals, including doctors, patients, the public, PAs and AAs and organisations from across the four countries of the UK who shared their views.
We carefully considered all feedback, and this will help to make sure that our approach to regulation supports patient safety.
The final rules, standards and guidance will be approved by our Council before regulation begins.

So you’re telling me that NHS Trusts, well known for their generosity and proactiveness when it comes to Dr pay, have NOT managed to ensure their residents are paid their backpay on time despite having months and months to plan ahead for this inevitable outcome?
Shock horror.
Trusts have had since August 2024 to get their act together and meet their backpay obligations. These delays are unacceptable.
This attempt to fob off responsibility and try and delay payment is typical of how NHS Trusts treat resident doctors with blatant disregard.
Hey @wesstreeting , can you please pause the GMC PA rollout of regulation planned for December until the independent review is done? It makes no sense otherwise.
Also note the GMC website does not currently distinguish MAPs on a separate filtered charter. It is all a mess.
We welcome the Department of Health and Social Care’s independent review into how physician associate (PA) and anaesthesia associate (AA) roles are deployed across the health system. It’s vital that we all work together to provide clarity and assurance for patients, doctors, PAs, and AAs alike.
We’re also pleased that the government has reaffirmed its commitment to bringing PAs and AAs into regulation from December 2024. Regulation is a vital step towards strengthening both patient safety and public trust in these professions.

Just saw someone from Joe Public reply to a Medical Consultant, a Dr who has been through every single possible iteration of Dr pay throughout training, with that’s not what a quick Google search says 👍’ after trying to explain how Drs get paid.
