هذه المجموعة الدوائية عظيمة

تدعى SGLT2

لماذا عظيمة ؟

١- تحسن السكر مهما كان نوع السكر
لان الية عملها تعتمد على طرح فائض سكر الدم في البول

٢- تعالج الكلى وتحميها بعد الله

٣- تحمي الشرايين والقلب من الجلطات

٤- تساعد في التحكم في الوزن وقد تقلل الوزن بعض الشيء

٥- لا تسبب بحد ذاتها انخفاض في سكر الدم

٦- قد تحسن الضغط قليلا

٧- قد تساعد في دهون الكبد قليلا

٨- قد تقلل من ارتفاع حمض اليوريك والحصوات الكلوية المرتبطة به

لكن .. يوجد جانب آخر مهم نتوعى حوله :

١- بشكل عام انصح بوقف هذه المجموعة الدوائية مؤقتا وقت المرض الشديد
مثل اصابة بالإنفلونزا / التهاب بول / التهاب رئة / عمل جراحة
لخطورة حصول الجفاف + حموضة الدم

٢- التوظيف المناسب
ماينفع يكون شخص سكره التراكمي مرتفع جدا ويضاف هذه الدواء لحاله لضبط السكر ، لانه بيعمل ٢٤ ساعة انه يطلع كل السكر في البول وهذا راح يسبب جفاف وهلاك في كثرة التبول . مفترض يضاف كطبق جانبي وليس وجبة رئيسية .

٣- في حالة تكرار حصول التهابات مسالك بولية / التهابات مهبلية ، أستشير طبيبك حول تبديل هذا الدواء

٤- في السكري النوع الاول ، يعتبر غير مصرح لوجود خطورة حصول حموضة الدم
لكن قد يصرف في ظروف خاصة off label
لذلك استشير طبيبك

٥- لو انت شاك في نوع السكري مثلا مو متاكد نوع اول او نوع ثاني ، بلاش تصرفه
لخطورة حموضة الدم

٦- قد تحصل حموضة الدم بدون ارتفاع في سكر الدم مع هذه الادوية نسميها

فلو انت تشتغل في طواريء خليك مصحصح لهذي النقطة ولا تعتمد على مستوى السكر فقط في حالة اشتباه في حموضة الدم ، ولابد نتاكد من (الكيتونات في الدم )

ليه؟
