Every seasoned physician, particularly in fields like internal medicine or cardiology, gradually develops a sixth sense—an intuitive ability shaped by years of clinical experience.

Yesterday, while I was in my consultation room, I heard a patient being wheeled into the cardiac scanning room. I noticed his cough, his breathlessness, and something about the way he presented triggered immediate concern. These were not just routine symptoms—they were ominous. My instincts told me this was serious.

I left my chair without a moment’s delay and went straight into the scan room. I instructed the technician not to make him lie down for the scan; we performed it with the patient seated. Immediately afterward, he was wheeled out, and I alerted the emergency room, which is about ten minutes from my clinic. I told them clearly: “Prepare for immediate intubation as soon as he arrives.”

Interestingly, the echo technician was surprised by the urgency in my response—especially since the patient’s room air saturation and blood pressure were within normal limits. But clinical instinct often sees what machines don’t.

The ER team intubated him promptly, and we moved him straight to the cath lab for an emergency angioplasty. This morning, when I visited the ICU, he had already been extubated. He recalled everything that had happened, right from the moment he entered the clinic to the life-saving procedure that followed.

Later, when the technician saw the angiogram, she understood why I had hurried him out of the scan room—despite not even completing optimal imaging.

The takeaway is this: time is muscle, and the sixth sense that comes with experience can mean the difference between life and death.
