Morton’s neuroma is a painful condition caused by thickening of the tissue around a digital nerve, most commonly between the third and fourth metatarsal heads, leading to sharp, burning pain or numbness in the forefoot. Diagnosis is typically clinical, based on localized tenderness and reproduction of symptoms with compression tests such as Mulder’s click or the foot squeeze test, sometimes supported by ultrasound or MRI. Treatment options range from footwear modification and corticosteroid injections to surgical excision of the affected nerve in refractory cases.

The Forced Shoulder Abduction and Elbow Flexion Test evaluates for SLAP tears of the shoulder. The examiner brings the patient’s shoulder into maximal abduction, then flexes the elbow so the hand moves down behind the head toward the opposite shoulder. A positive test is indicated by pain at the posterosuperior aspect of the shoulder during forced maximal abduction, which is relieved or diminished when the elbow is flexed. Sensitivity and specificity is 67%!

The FABER or flexion, abduction and external rotation test evaluates for hip and sacroiliac disease. The tested leg is placed in a “figure 4” position, with the hip flexed, abducted, and externally rotated so that the ankle rests on the opposite thigh. The examiner stabilizes the contralateral pelvis at the anterior superior iliac spine while applying gentle downward pressure on the tested knee. A positive test is indicated by pain or limited range of motion. Diagnostic characteristics vary widely!
