Do low-dose glucocorticoids prevent major relapse during #AAV maintenance?

In 171 patients with GPA or MPA followed for a median of 7.4 years:
❌ Glucocorticoids did not reduce major relapse risk
✅ Rituximab was associated with lower relapse risk
⚠️ Higher glucocorticoid exposure was associated with more damage accrual and hospitalisations

These real-world data support tapering and discontinuing glucocorticoids after remission whenever feasible.

El dilema del bicarbonato en la ERC avanzada: ¿Frenar la progresión o sobrecargar el corazón? 🧪⚖️

Pautar bicarbonato de sodio de forma universal ante cualquier acidosis metabólica leve en la ERC avanzada exige precaución hemodinámica.

Un ensayo clínico aleatorizado de fase 3 publicado en @Kidney_Int demuestra que, si bien la suplementación alcalina oral estabiliza la caída del eGFR en pacientes con ERC estadios 4-5, en aquellos con disfunción cardíaca subyacente incrementa de forma significativa el riesgo de hospitalización por insuficiencia cardíaca debido a la sobrecarga sódica concomitante.

En la consulta: si tu paciente roza el peso seco o tiene cardiopatía estructural, considera prioritario el ajuste dietético y optimiza diuréticos antes de forzar la carga de sodio exógeno.
