Regional Health Responsibility and the Cost of Migration on South African Healthcare🇿🇦🩺:

When Zimbabwean MP Mutsa Murombedzi asked how his government plans to treat HIV-positive citizens returning from South Africa, it wasn’t just politics. It exposed a massive regional blind spot. Uninterrupted treatment is a human right, but who pays for it?

South Africa runs the world’s largest public HIV treatment programme. By law, anyone qualifies, citizen or not. It’s an incredible humanitarian effort, but it has pushed our local public healthcare system to an absolute breaking point.

Let’s talk numbers. HIV treatment costs tens of thousands of Rands per patient over time. When neighbouring countries underfund their own health sectors, South African taxpayers silently absorb the bill for the entire region. It’s unsustainable.

Regular migration requires strict medical screenings (like TB tests) so hospitals can plan budgets. Irregular migration bypasses this entirely. The issue isn't the people; it's the total lack of data. You cannot budget for unmapped health needs.

The result? Local clinics face devastating drug stockouts, doctors suffer severe burnout, and everyday South Africans struggle to access basic care. Compassion without a budget is just a crisis waiting to happen. 💔

This is no longer a local issue it’s an African Union (AU) problem. One country cannot be expected to act as the medical safety net for the entire SADC region without financial and systemic backing.

The AU needs to step in now with an Equitable Resource Sharing Framework. Sending nations must co-fund the healthcare of their citizens abroad, and member states must finally hit their Abuja Declaration goal of 15% budget allocation to health.

Virus transmission doesn't care about borders, and neither should health tracking. We need secure, cross-border digital health registries to ensure patient continuity, stop drug resistance, and save lives safely.

True regional solidarity isn't about letting one nation carry the entire financial burden until it collapses. It’s about sharing the responsibility for long-term care as fluidly as the migration itself. 🇿🇦🤝🌍
