🧠“右上肺静脈のablationでSVCまで焼ける？”—PFAによるSVCインパクトの実態
🔷 PulseselectでもFarapulseでも…右上PVIがSVCに届いてる？
なんと8割でSVCに新たな低電位領域が出現。
しかも10人に1人はSVCが全周性にablation

🔍 論文要点まとめ

対象：68名（Varipulse:17, PulseSelect:26, Farapulse:25）
RSPV PVI後に右房マッピングを実施

SVCに新規low voltage area出現：82.4%

SVC全周性のlow voltage area：14.7%

システム別SVC影響率：
　↪ Varipulse：52.9%
　↪ PulseSelect：96.2%
　↪ Farapulse：88.0%（p = 0.0009）

RSPVとSVCの距離、**SVCの形（湾曲＋両端狭小）**が強く関連
Varipulseで頻度が最も少なかった。

🧠 考察ポイント

特にSVCが“月型”(詳細は本文を参照)に変形していると広範囲SVC LVAの確率が約4倍
意図せずしてSVCへ影響が出ることで将来のSVC関連ATやSNRTに繋がる可能性も？
“意図しない焼灼”が起きるPFAの深達性と方向性、注意する必要あり
