The most expensive part of a hospital bill may never touch the hospital at all.

A patient admitted for care has no way of knowing whether the price on a medical consumable reflects its actual cost or a markup fixed long before it ever reached the ward. That gap in information is, at its core, a public health issue.

A survey of hospital consumables in Maharashtra found an IV infusion set with a trade price of ₹11.05 carrying a printed MRP of ₹325 ; a markup of 2,841%. A syringe procured at ₹6.75 carried an MRP of ₹57.20. A catheter procured at ₹29.41 carried an MRP of ₹310.

These are not elective purchases. Patients cannot compare prices, seek alternatives, or question a number printed on a box while receiving care and the MRP itself is often fixed upstream by manufacturers and distributors, disconnected from the trade price by a wide, unexplained margin. The result is a system where the party bearing the cost has the least information to evaluate it.

The regulatory gap is structural: scheduled medicines are capped under the Drugs (Prices Control) Order, 2013. Most medical devices and consumables are not leaving both the pricing and the information around it almost entirely unmonitored.

A review of these findings and clear guidelines on the permissible gap between trade procurement price and declared MRP have been recommended to the Department of Pharmaceuticals and the NPPA ; a step toward closing not just a pricing gap, but the information gap patients are left to bear alone.

The OECD conducts an assessment called PISA that measures the reading, science, and maths skills of 15-year-olds. The latest results are quite dismal. Across much of the world, kids are getting worse at reading and maths, and the decline isn't small.

COVID probably explains some of this, but there seems to be a longer-term trend at play. And now there's AI.

I can see a lot of this in myself. Pretty much every post I write now goes through ChatGPT, Claude, etc. Because I know AI will fix my grammar and typos, there's less incentive to get them right in the first place. And over time, I genuinely feel like my writing without AI is getting worse.

I keep asking K (CTO) about coding and whether something similar is happening with our tech team. His answer isn't very reassuring either. 😬

All of this makes me wonder what the right approach is with kids. On the one hand, you don't want them outsourcing thinking, writing, problem-solving, etc., before they've learnt how to do these things themselves by making mistakes. On the other hand, if they're growing up in a world where AI will be a big part of their lives, you want them to be good at using it.

The problem with humans is that, given a choice between an easy thing and a hard thing, we'll almost always choose the easy path. The same applies to AI. Even if it can help us think and do more, I think most of us will end up outsourcing a lot of our thinking and doing to it.

I don't know how many here have watched Idiocracy, but sometimes it feels like we are accelerating towards it. 😬

PS: Idiocracy is about an average man who wakes up 500 years in the future to discover that human intelligence has declined so much that he is now the smartest person alive.🙂

రాజమహేంద్రవరం నగర కాలుష్యంలో సగం ఆంధ్రా పేపర్స్ లిమిటెడ్ నుంచే వస్తుందని అధికారిక గణాంకాలు సైతం చెబుతున్నాయి. రాజమహేంద్రవరం నగరం మొత్తం దాదాపు 70 మిలియన్ లీటర్ల ద్రవ వ్యర్థాలు ఉత్పత్తి అవుతుంటే, మీరొక్కరే 35 మిలియన్ లీటర్ల వ్యర్థాలు ఉత్పత్తి చేస్తున్నారు. మీరు విడుదల చేస్తున్న వ్యర్థాల్లో పరిమితికి మించి కాలుష్య కారకాలు ఉన్నట్టు నిర్ధారణ కూడా అయ్యింది. మీరు వ్యర్థాలు విడుదల చేసే లగూన్స్ దగ్గర సేకరించిన నమూనాలను పరీక్షించగా, లీటర్ నీటికి 36 మిల్లీగ్రాముల బయో కెమికల్ ఆక్సిజన్ డిమాండ్ (బీఓడీ) ఉంది. ఇది నిర్ణీత ప్రమాణాల కంటే 20 శాతం అధికం. ఫాస్ఫేట్ 5.3 మిల్లీగ్రాములు 6 శాతం అధికంగా ఉంది. సల్ఫైడ్స్ 3.4 మిల్లీగ్రాములు ఉన్నాయి. కేంద్ర కాలుష్య నియంత్రణ మండలి ప్రమాణాలతో పోలిస్తే.. ఇది 70 శాతం అధికం. మీరు విడుదల చేసే కాలుష్యం కారణంగా నీటిలో ఆక్సిజన్ శాతం తగ్గి జలచరాల మనుగడకు ఇబ్బంది కలుగుతుంది. గోదావరి నది సహజ పర్యావరణ వ్యవస్థ దెబ్బ తింటోంది. దుర్గంధాలు వెదజల్లుతూ, నీటి నాణ్యత తగ్గి ప్రజల ఆరోగ్యాన్ని దెబ్బతీస్తూ, భవిష్యత్ మానవాళి మనుగడను ప్రశ్నార్థకంగా మారుస్తోంది. భూగర్భ జలాలు కలుషితం కాకుండా తగిన చర్యలు తీసుకోవాలని క్షేత్ర స్థాయి పర్యటనలో నేను సూచనలు చేశాను. వాటిని కూడా మీరు పాటించలేదు.

దీంతో కాలుష్య నియంత్రణ మండలి అధికారులు తక్షణం ఆంధ్రా పేపర్స్ లిమిటెడ్ కి సమన్స్ జారీ చేసి నిబంధనల మేరకు తగిన చర్యలు తీసుకోవాలని ఆదేశిస్తున్నాను.
ప్రభుత్వం తరఫున అడ్వకేట్ జనరల్, అదనపు అడ్వకేట్ జనరల్‌తో పాటు ప్రభుత్వ న్యాయవాదులు, స్టాండింగ్ కౌన్సిళ్లు ఇటువంటి ఉల్లంఘనలకు సంబంధించిన కేసుల విచారణను త్వరితగతిన పూర్తి చేసేలా అవసరమైన చర్యలు తీసుకోవాలని సూచిస్తున్నాను. స్వచ్ఛ గోదావరి-పవిత్ర పుష్కరాలు థీమ్ తో మన గోదావరి నదిని కాలుష్యం నుంచి కాపాడేందుకు ప్రభుత్వం తీసుకుంటున్న చర్యలకు పర్యావరణ వేత్తలు, పౌర సమాజం నుంచి మద్దతు కోరుతున్నాము. ప్రజా ప్రయోజనాలు, పర్యావరణ పరిరక్షణకు కూటమి ప్రభుత్వం అత్యంత ప్రాధాన్యతాంశంగా ముందుకు తీసుకువెళ్తుందని తెలియజేస్తున్నాను.
