Amal Khalil, a Lebanese journalist, was targeted today by the IDF in South Lebanon, together with other colleagues. Two were killed, one injured and Amal is still under the rubble. The Red Cross tried to extract her but they were targeted too by the Israelis and had to retreat.

Ada indikasi di mana susu formula dibutuhkan loh, misalnya di kondisi ASI gak cukup atau berisiko berbahaya untuk bayi dan anak.

(Maaf sambil marah-marah, karena bisa berisiko untuk anak kalau pernyataan kontra sufor ini ditelan mentah-mentah!)

Jadi ada kondisi di mana penggunaan susu formula itu diindikasikan loh. Panduannya juga ada, salah satunya adalah "𝗔𝗰𝗰𝗲𝗽𝘁𝗮𝗯𝗹𝗲 𝗠𝗲𝗱𝗶𝗰𝗮𝗹 𝗥𝗲𝗮𝘀𝗼𝗻𝘀 𝗳𝗼𝗿 𝗨𝘀𝗲 𝗼𝗳 𝗕𝗿𝗲𝗮𝘀𝘁-𝗠𝗶𝗹𝗸 𝗦𝘂𝗯𝘀𝘁𝗶𝘁𝘂𝘁𝗲." Kondisi tersebut antara lain:

1. 𝗕𝗮𝘆𝗶 𝗯𝘂𝘁𝘂𝗵 𝘀𝘂𝘀𝘂 𝗳𝗼𝗿𝗺𝘂𝗹𝗮 𝗸𝗵𝘂𝘀𝘂𝘀 𝘆𝗮𝗻𝗴 𝘁𝗶𝗱𝗮𝗸 𝗯𝗶𝘀𝗮 𝗱𝗶𝗴𝗮𝗻𝘁𝗶𝗸𝗮𝗻 𝗼𝗹𝗲𝗵 𝗔𝗦𝗜
- Galaktosemia klasik, di mana bayi butuh formula bebas galaktosa. Kalau dipaksakan diberikan ASI, maka bayi bisa mengalami berbagai kondisi meliputi gagal hati, kerusakan otak, bahkan kematian dalam hitungan hari, dengan angka kematian tinggi mencapai 75% tanpa terapi.

- Maple syrup urine disease, dimana bayi butuh formula bebas asam amino leusin, isoleusin, valin.

- Fenilketonuria (PKU), dimana bayi butuh formula bebas fenilalanin.

2.  𝗕𝗮𝘆𝗶 𝘆𝗮𝗻𝗴 𝗯𝘂𝘁𝘂𝗵 𝗳𝗼𝗿𝗺𝘂𝗹𝗮 𝘀𝗲𝗯𝗮𝗴𝗮𝗶 𝘀𝘂𝗽𝗹𝗲𝗺𝗲𝗻 𝗔𝗦𝗜
- Bayi berat badan lahir sangat rendah (BBLSR) (<1500 g) atau prematur <32 minggu.
- Bayi risiko hipoglikemia yang tidak respons ASI optimal
- Bayi dengan dehidrasi berat, hiperbilirubinemia berat, atau penurunan BB >8-10% yang tidak teratasi dengan ASI.

3. 𝗞𝗼𝗻𝗱𝗶𝘀𝗶 𝗶𝗯𝘂 𝘆𝗮𝗻𝗴 𝗺𝗲𝗻𝗴𝗵𝗮𝗿𝘂𝘀𝗸𝗮𝗻 𝗽𝗲𝗺𝗯𝗲𝗿𝗶𝗮𝗻 𝘀𝘂𝘀𝘂 𝗳𝗼𝗿𝗺𝘂𝗹𝗮
-Ibu sakit berat, misalnya sepsis atau penyakit lain yang tidak memungkinkan menyusui)
-Ibu yang konsumsi obat atau dalam terapi tertentu (kemoterapi, radioaktif, psikotropika tertentu)
-Ibu HIV+ tanpa ARV pada konteks di mana formula AFASS (aman, feasible, affordable, sustainable, safe) — rekomendasi WHO
-Ibu HTLV-1/2 positif

INGAT! Ada perbedaan fundamental antara "mengutamakan ASI " dengan "menolak sufor pada bayi yang butuh secara indikasi medis".

Ibu yang milih ASI eksklusif itu hebat.
Ibu yang memilih sufor karena indikasi medis juga sama hebatnya.
Dua-duanya adalah ibu yang berjuang buat anaknya.

Yang gak hebat cuma satu yaitu orang yang menyebarkan framing tanpa bukti ilmiah terus bikin ibu-ibu lain merasa bersalah padahal anaknya memang butuh susu formula karena kondisi tertentu.
