When I take the PATH to work for a 3p-11p shift, I often get a Lyft home. Driver last night was listening to a lengthy YouTube on the history of impalement. I also listen to lots of weird shit, but I turn it off even in the Dunkin drive thru, nevermind with a passenger!
Made a life-saving diagnosis today.
Patient did great.
But I’m kicking myself that I didn’t get it 20 minutes sooner.
All the signs were there but it was an atypical presentation—patient looked far better than is usual with this diagnosis.

I had a postpartum hemorrhage wy 1st. I’m sure it was distressing to my spouse—I handed kid off to him so I could focus on not dying— but ultimately I was the one at risk of death. Especially in light of the recent overturn of Roe, I DGAF about the nonpregnant partner’s feefees.

Like many other ADHDers I find that high-stress situations sometimes lock my brain into focus. I excelled at exams at school. I am great in a crisis.
But like… it should be obvious that
“Be in high-stress situations where you have to focus 122%” is not workable long term.
Contrarian viewpoint: 24h+ rotations as a medical student helped direct me away from fields where that would be required. I’m not sure that there’s any better way to sort out who can manage that well and who cannot.
Why do hospitals make students do 24h shifts if they’re not interested in specialties or residency programs that have 24s

Sometimes you just know, even if your only interaction is monthly department meetings, that someone is bad news. You then watch in horrified fascination as they try to destroy a major reproductive rights org.
Years later they use a pandemic as a way to continue their grift, getting a national platform for their bad takes, despite their dilettante-ish record of failures.
I learned this as “The longer you stay, the longer you stay” and it is definitely true in EM. Give a good sign out and then get the hell out, or you will get pulled into “just one more thing” ad infinitum.
