𝗕𝗿𝗲𝗮𝘀𝘁 𝗰𝗮𝗻𝗰𝗲𝗿 𝗺𝗼𝗿𝘁𝗮𝗹𝗶𝘁𝘆 𝗳𝗲𝗹𝗹, 𝗯𝘂𝘁 𝗼𝘃𝗲𝗿𝗮𝗹𝗹 𝘀𝘂𝗿𝘃𝗶𝘃𝗮𝗹 𝗱𝗶𝗱 𝗻𝗼𝘁 𝗶𝗺𝗽𝗿𝗼𝘃𝗲. 🎀

A 20-year unplanned subset analysis of the EORTC 22922/10925 trial adds important nuance to the role of internal mammary and medial supraclavicular lymph node irradiation in node-negative breast cancer.

Among patients with pN0 disease and long-term follow-up of more than 22 years, IM-MS irradiation was associated with lower breast cancer mortality:

🔹 10.0% with IM-MS irradiation
🔹 14.2% in the control group
🔹 HR 0.70; p=0.010

But overall survival was nearly identical:

🔹 69.0% with IM-MS irradiation
🔹 68.4% in the control group
🔹 HR 0.98; p=0.77

The study also reported a late signal of higher non-breast cancer mortality and more lung fibrosis with IM-MS irradiation.

The message is not that regional nodal irradiation has no value.
It is that in node-negative breast cancer, benefit must be weighed carefully against long-term risks, baseline recurrence risk, patient selection, and radiation technique.

Very long-term data matter.

So does precision.
