Per 2022 5th WHO:
If you have a histologically differentiated tumor
- Papillary carcinoma
- Follicular carcinoma
- Oncocytic carcinoma
with tumor necrosis (punctate to extensive, excluding FNA changes) or ≥5 mitoses per 2 mm², then you can call it high-grade differentiated thyroid carcinoma.
