If you have a hard time understanding the emotions of someone is that not a lack of empathy? Same goes for autism too, lack of ToM, characterized by lack of empathy

They tend to feel immense emotional pain and when that pain is triggered it then overrides that strong empathy that they have. Ex: Amber Heard slicing a finger
