The Ischemic Cascade; why ischemia whispers before it screams
Myocardial ischemia is a process, not an instant event. Long before a patient feels chest pain, the heart is already struggling.
It begins with reduced regional myocardial perfusion, creating a mismatch between oxygen supply and demand. At the cellular level, metabolism shifts as mitochondria fail to meet energy needs. Only later do we see LV diastolic dysfunction, followed by systolic dysfunction, ECG changes, and finally the symptom patients recognize, angina.
This sequence explains a common clinical pitfall: waiting for chest pain or ECG changes means we are detecting ischemia late.
That’s why stress testing with myocardial perfusion imaging is more sensitive than approaches relying solely on wall-motion abnormalities or ECG changes. Perfusion defects and metabolic disturbances occur first, often silently.
Clinical takeaway:
Ischemia doesn’t announce itself early. If we only listen for pain or ECG changes, we miss the beginning of the story.
