3 things I wish every clinician knew about pharmacovigilance:

A safety report is not just a case to be processed.
Behind every ICSR is a patient, a clinical event and a piece of evidence that may contribute to understanding a medicine’s risk.
Pharmacovigilance isn’t just paperwork.
It’s patient safety in slow motion.
The hardest part of progress is that it often looks like nothing is happening.
One workout doesn’t change your body.
One book doesn’t change your thinking.
One good decision doesn’t change your life.
But repeated often enough, ordinary actions become extraordinary results.
If my ticket says 7:30 PM, I expect the movie to start at 7:30 PM.
Not 7:50.
I understand cinemas need advertising revenue. But why should the customer’s time be the currency?
If 20 minutes of ads are unavoidable, just call it a 7:50 PM show.

I agree ORS isn’t needed for every workout, but “avoid ORS with exercise” is too categorical.
ORS is designed primarily for diarrhoeal dehydration, but its glucose sodium mechanism isn’t specific to diarrhoea. After prolonged exercise or heavy sweating, ORS can very well be used

It’s major role
1. To restore fluids and electrolytes lost in diarrheal illness and other pathological fluid loss.
It's not formulated, labeled, or intended as an exercise drink.
Taking it 'with every workout' is a misuse of a medicine.
This is how ORS actually works.
ORS exploits a specific absorption pathway
- glucose and sodium ride the same channel (SGLT1) to drag water into your blood.
It's incredibly effective when you've lost fluid from the gut.
But during normal training, your sweat losses are different, your stomach isn't the problem, and you don't need that mechanism.
That's literally what sports drinks and electrolytes are for.
Even those are only needed for longer, sweat-heavy sessions.
You can actually go to a online sweat calculator and estimate this.
• Sodium loss
• Whether plain water is enough
• Or whether electrolytes
makes sense for your body

ORS and electrolytes are different products for specific scenarios.
And by all means spend on high quality gym membership, clothes and shoes.
All are investments for better health.
The older I get, the more I value curiosity over certainty.
Certainty makes you stop looking.
Curiosity makes you ask one more question.
And sometimes, that one question changes everything.
For those who are not diabetic or clinically obese, diet & exercise should be first option. GLP-1 alters metabolic function n has neurological signalling which in the long term may have side effects. There is not much long term data on chronic use of GLP-1s.

NSAIDs (ibuprofen, naproxen, aspirin) + blood thinners = severe bleeding risk.
This risk rises further if you combine two NSAIDs or add alcohol.

On a different note, Air india express new food menu is great.
In few years there would be no seats or jobs for our kids as general category. Also going by current trends it would be hard to survive in a foreign country. What is the way forward 🤔
The MP govt has raised reservation in direct recruitment to 73%. The remaining 27% isn’t earmarked for general category, it’s open to all.
The matter went to the SC, where the MP govt, in its affidavit, made arguments like these:

All of this can be disproven with even a little scrutiny, yet it’s shocking to hear such statements from a BJP govt. One would expect them to bridge divides and unite Hindus, this does the exact opposite. And if BJP truly wants to follow this path, why stop short? Why not reserve 100% for Bahujans? Why not close the doors entirely to the general category and be done with it for once and all?
