3. Lack of fuel supply to the engines leads to fail.

Someone asked how did both the engines fail?..Only one engine might have failed. So
1. All the loads fall on the second engine, hence that was also failed.
2. The pilot might have accidentally shut down the good engine instead of the failed engine.

When you say engine failure.. which part of the engine you suspect of malfunctioning! Do you know how many parts in the GE next generation engine of Boeing 787? Total 1.1 million parts to form one GEnx engine.

Bird strikes, fuel starvation, fuel leaks &engine damage could all potentially cause an aircraft’s engines to shut down.
Another issue: the pilot might have switched off the wrong engine.
Was there any periodic insp done?
