India’s first completely free rural medical college, the Sri Madhusudan Sai Institute of Medical Sciences and Research (SMSIMSR), is training a new generation of doctors to serve underserved communities instead of pursuing profit. Founded in 2023 in rural Karnataka, the college provides world-class education, clinical training, and housing at no cost to students from marginalized backgrounds.
Students begin hands-on hospital work from their first year and are taught with a focus on compassion, ethics, and community service. With over 1 million outpatient visits and 29,000 surgeries already performed for free, SMSIMSR is reshaping healthcare access in India and building a scalable model of value-based, inclusive medical education
