Agree that medicine is difficult and we can’t allow shortcuts that harm patients. But I also think we take advantage of learners in the name of “better training” by putting them through years of school, residency and endless fellowships all for low pay. Two things can be true: (1) medicine is hard and takes a long time to learn and (2) some of our current training standards are unnecessarily long and could be shortened.

@OBGaYN learned something new today — this is great to know! there are 1 or 2 other exceptions I’d make to using an ACEi, but mainly arguing here as an up-front first-choice antihypertenive ARB > ACEi (except in breast feeding!)

"No. Just want to help the sick."
"What about community service?"

"No. Just want to help people with cancer, diabetes, other conditions. I know a lot of people like this. They need good doctors."
"Hmmm ...."
