On 16th July a federally regulated exchange began taking bets on whether specific cancer and Alzheimer’s drug trials will succeed.
One contract at launch asked whether AR1001 would meet its primary endpoint in early Alzheimer’s disease.
Kalshi runs the markets with a data firm called AppliedXL. Polymarket lists comparable contracts.
Kalshi says it lists a trial only after enrolment closes, so a visible price cannot influence recruitment. It says it will require employment verification from traders in these markets, and that its existing ban on trading with material nonpublic information applies. Those are the company’s descriptions of its own pilot.
Joshua Pederson, a Boston University professor whose 12 year old son is enrolled in a cancer trial, learned the markets existed and objected to the whole category. He told NPR that a trial failing means “people are going to suffer, people are going to die, people are going to have one fewer clinical option available to them.” NPR does not name his son’s trial, and no reviewed source shows it is listed anywhere.
Kalshi’s defence is that equity markets have always let investors profit from failed trials, at far larger scale. That is true, and it is also the point. Owning a share prices a company. This prices one treatment.
Each contract names the public document it settles against.
For trial outcomes, that document is the registered primary endpoint on https://t.co/opmQDedzLK. The same record that says whether the drug worked.

An expert witness hired by 3M typed that into ChatGPT. The prompt sits in a court record made public this month, lowercase and unedited.
The blast happened in Houston on 24th January 2020. It killed 3 people and damaged or destroyed hundreds of homes.
An earlier prompt in the same record asked ChatGPT to create an "exceptional expert witness report" defending 3M. Later in that same chat, ChatGPT warned him that calling 3M "0% responsible" would be an easy target for opposing counsel.
Separately, a Harris County jury awarded $61.5 million to 24 plaintiffs. 3M says it disagrees with the verdict and will appeal.
The issue is not that a machine drafted an expert report.
The conclusion was typed first. The reasoning was generated afterwards to arrive at it.
