This meta-analysis of 65 studies involving 13 971 patients compared transanal with laparoscopic total mesorectal excision (TME) for rectal cancer. Transanal TME was associated with better surgical and oncological outcomes but a higher risk of major low anterior resection syndrome. The findings highlight the need to balance functional outcomes with oncological benefits in future research.

Long-term effects of Hirschsprung disease in adults: meta-analysis and patient-level regression study

This systematic review assessed long-term outcomes in patients with Hirschsprung disease (HSCR), focusing on bowel, urological, and sexual function, fertility, and quality of life. Patients with HSCR were more likely to experience constipation, soiling, and lower quality-of-life scores (especially women and those with severe disease), with physical role limitations and poorer Gastrointestinal Quality of Life Index scores being prominent. These findings emphasize the need to address the functional impairments and challenges faced by patients, particularly women and those with more severe forms of the disease, during their transition to adult care.

Management and risk factors for colonic volvulus: retrospective national cohort study

Surgery should be recommended for colonic volvulus where co-morbidity, patient preference, and functional status allows. Surgically unfit patients have poorer outcomes. Elective sigmoidectomy after endoscopic detorsion is preferred as it carries the lowest mortality risk. Necrotic bowel, dependency, and co-morbidities predict death for both sigmoid and caecal volvulus.

Characteristics and prognoses of patients with lateral pelvic lymph node (LPN) metastases without mesenteric lymph node (meso-LN) metastases are poorly studied. This retrospective, multicentre study aimed to explore patterns of LPN and meso-LN metastases and surgical outcomes in 407 patients with rectal cancer. Patients with metastasis confined to the LPNs have comparable pathological features and prognoses to those with metastasis confined to the meso-LNs; such disease can be managed and treated in the same way as regional lymph node metastasis.

Chronic anastomotic leakage (AL) is the most common cause of pouch failure after restorative proctocolectomy with ileal pouch–anal anastomosis for ulcerative colitis. Early diagnosis and treatment of AL diminishes the negative effect of AL after ileoanal pouch surgery. Proactive anastomotic assessment is mandatory to enable early diagnosis and management, especially in patients at increased risk of silent leaks, such as those undergoing a three-stage approach.
