The PhD, academic doctorate lineage used the Doctor title first for several centuries. The word itself is a Latin verb docēre, meaning “to teach”.
Then medical practitioners adapted it, as it stands, many including the medical doctors assume they claimed it first. They were initially called Physicians (Medical Doctors), Apothecaries (Pharmacists), and Surgeons.
Practicing Physicians began using “Doctor” only in the 18th and 19th centuries to distinguish themselves from quacks, borrowing the prestige associated with university academics (The PhDs).
Dentists, Vets and Pharmacists all adapted the title between the 20th and 21st century.
Then PharmD follows same North American professional doctorate model as the US MS, designated to reflect a doctoral level clinical training.
Now, I’m not a fan of the title and the confusion it brings, and believe me, even after earning a PhD, I will reject being called a Dr, I will rather be called Pharm. Yusuf Dauran, but we are strictly respecting historical hierarchy, PhD holders are the only Doctors.
